Doctor visit checklist for an elderly parent

A good appointment starts before you arrive and isn’t over until the family knows what changed. Use this checklist to walk in prepared and walk out with a clear plan everyone can follow.

Before the visit

  • Bring the current medication list (including OTC and supplements).
  • Write down what’s changed since the last visit — symptoms, mood, sleep, appetite, falls.
  • List your top 3 questions first, in case time runs short.
  • Note who is coming and who is driving.
  • Bring insurance cards and a photo ID.

Questions to ask

  • What is this appointment telling us about how they’re doing overall?
  • Are any medications new, changed, or safe to stop?
  • Do any medications interact — including the OTC ones?
  • What symptoms should prompt a call, and what would be an emergency?
  • What’s the plan until the next visit, and when is that?

  • Update the medication list the same day if anything changed.
